Metastatic intracranial malignant melanoma is common in Caucasian but in oriental population, intracranial melanoma is a rare type of brain tumor regardless of primary or metastatic lesion. We present a case of metastatic intracranial melanoma in a 66-year-old female with slurred speech and memory impairment. Brain computed tomography scan and magnetic resonance imaging showed a well enhancing mass lesion in the left basal ganglia resembled an high grade glioma. Left frontal craniotomy with grossly total excision was performed. Histopathological diagnosis was malignant melanoma. MR image of melanoma is a characteristic feature with high T1 and low T2 intensity lesion. However, amelanotic type shows T1-low & T2-high intensity lesion, should be included among the differential diagnosis of intracranial high grade malignancy in MRI. Surgical excision of brain metastatic melanoma, although not curative, may increase survival time in patients.
